What Is Jardiance?
Jardiance (empagliflozin) is prescribed for the treatment of type 2 diabetes and to lower the risk of heart attack or stroke in people who also have heart failure or certain risk factors for heart disease. Jardiance is a sodium glucose co-transporter 2 (SGLT2) inhibitor. As such, it keeps the kidneys from reabsorbing glucose and increases the excretion of glucose in urine.

- Age 18 or over
- Commercial health insurance
- Prescription coverage for Jardiance, but below 100%
Being enrolled in a government-funded insurance program like Medicare, Medicaid, CHAMPUS, VA insurance, or other similar plan makes you ineligible to receive a Jardiance copay card. There may be an exception for individuals with Medicare Part A or Part B who also have commercial prescription coverage for Jardiance.
