How Medicare’s New $2,000 Prescription Drug Cap Works in 2025

Understanding the $2,000 Drug Cap  

The drug cap allows Medicare Part D beneficiaries to avoid any further out-of-pocket expenses for their medications once they reach the annual $2,000 threshold. Before the passing of this new regulation, there were no out-of-pocket caps for Medicare Part D—which addresses prescription drugs—leading many seniors to spend fortunes each year on medications alone.   

The drug cap will cover all prescription drugs covered by Part D plans, including deductibles and copays, and it will apply to all beneficiaries of this plan, regardless of their income. This will benefit those with high medication costs and those who don’t qualify for the federal Extra Help program that might have otherwise covered some of these expenses.   

Keep in mind that the drug cap does not apply to drugs covered by Medicare Part B, including injectables and infused drugs. Part D plans may choose not to cover certain medications as well, and this means that the cap won’t apply to them.  

Aside from those who have Part D plans, the cap also applies to people who have a drug plan through Medicare Advantage.

Requires No Enrollment  

When hearing about this new rule, seniors are often worried about a long and complicated enrollment process, but that’s not the reality with this cap. You don’t have to sign up at all. The cap is automatically applied to your Part D plan, and the plan itself will track your spending, with the cap kicking in the moment you reach $2,000.
